All six ApoE genotypes.
Called with precision.
PCR-based SNP genotyping for ApoE at codons 112 and 158 — the ε2 / ε3 / ε4 workup required before Leqembi (lecanemab) and Kisunla (donanemab) dosing. Q-Analyzer software peak-calls each electropherogram and assigns one of the six ε2/ε2-to-ε4/ε4 genotypes automatically, eliminating manual gel interpretation. Compatible with every Qsep instrument using the S1 High Resolution Cartridge (C105202). Available for blood, saliva, and oral swab samples.
| Blood | C900400-48 | $432 ($9 / test) |
| Saliva | C903401-048 | $432 ($9 / test) |
| Oral Swab | C903402-048 | $432 ($9 / test) |

Two FDA-approved anti-amyloid drugs. Both gate dosing on ApoE.
Both anti-amyloid antibodies tie initiation and titration to the patient's ApoE ε4 status, citing elevated risk of amyloid-related imaging abnormalities (ARIA) in ε4 carriers and homozygotes.
Lecanemab (Leqembi)
Eisai / Biogen. FDA-approved 2024. Anti-amyloid mAb for early Alzheimer's disease. Prescribing information requires ApoE genotyping before initiation — ARIA-E risk is markedly higher in ε4 carriers than in non-carriers.
Donanemab (Kisunla)
Eli Lilly. FDA-approved 2024. Anti-amyloid mAb with modified-titration dosing tied to ApoE genotype — ε4 homozygotes receive a slower escalation schedule.
Pre-dose workup
Memory clinics, neurology practices, and reference labs are scaling ApoE genotyping volume to match anti-amyloid prescribing — and bundling ε4 status into pre-treatment screening panels.
Three alleles. Six genotypes. Multiple diseases.
Apolipoprotein E (APOE) is involved in blood-lipid metabolism and is associated with Alzheimer's disease, hyperlipidemia, and cardiovascular disease. Three alleles arise from SNP variation at codons 112 and 158, with one allele inherited from each parent.
| Allele | Codon 112 | Codon 158 | Notes |
|---|---|---|---|
| ε2 | Cys | Cys | May antagonize Alzheimer's risk. ε2/ε2 strongly correlated with familial type III hyperlipoproteinemia. |
| ε3 | Cys | Arg | Most common allele. Neutral for Alzheimer's risk; normal lipid metabolism. |
| ε4 | Arg | Arg | Alzheimer's risk increased ~3–5×; ε4/ε4 homozygotes ~5–15×. Higher LDL clearance failure, atherosclerosis risk. |

Three steps. Sample to genotype.
Amplify the ApoE locus, digest, and read the fragment pattern on Qsep — no agarose gel, no manual scoring.
1 · Amplify
PCR across the ApoE locus spanning the codon 112 and codon 158 SNPs. Runs on the Qamp Mini thermal cycler or any standard thermocycler.
2 · Digest
Restriction digestion cuts the amplicon into allele-specific fragments. Enzyme and reaction buffer included in the kit.
3 · Size & auto-call on Qsep
Run on the S1 High Resolution Cartridge (C105202). Q-Analyzer software peak-calls each electropherogram and assigns one of the six ApoE genotypes automatically — no manual gel interpretation. Reports export in PDF, Word, or Excel.
