Both anti-amyloid antibodies tie initiation and titration to the patient's ApoE ε4 status, citing elevated risk of amyloid-related imaging abnormalities (ARIA) in ε4 carriers and homozygotes.

Lecanemab (Leqembi)

Eisai / Biogen. FDA-approved 2024. Anti-amyloid mAb for early Alzheimer's disease. Prescribing information requires ApoE genotyping before initiation — ARIA-E risk is markedly higher in ε4 carriers than in non-carriers.

Donanemab (Kisunla)

Eli Lilly. FDA-approved 2024. Anti-amyloid mAb with modified-titration dosing tied to ApoE genotype — ε4 homozygotes receive a slower escalation schedule.

Apolipoprotein E (APOE) is involved in blood-lipid metabolism and is associated with Alzheimer's disease, hyperlipidemia, and cardiovascular disease. Three alleles arise from SNP variation at codons 112 and 158, with one allele inherited from each parent.
